Audition Request Form

Prospective students can complete and submit an audition form here.  The last audition date for the 2008-9 academic year is May 1, 2008. 

MUP (Private Lesson) Form for Music Majors

Attention current WSU Music Majors:  You may now download, complete and print the MUP form (Applied Music Request Form). The completed form must be printed, signed by your advisor and the Associate Chair (Dr. Norah Duncan IV), and returned to the main music office.  Applied Music Request Form (form requires Adobe Acrobat Reader)

Note: You must complete each course in the College of Fine, Performing and Communication Arts with a grade of C (2.0) or better. This also includes all transfer credits that may be applicable to the major programs. Major coursework will be defined by the department where the major program resides.
